С другом, поездом, врачом: with a friend, by train, as a doctor. The Russian instrumental gathers company, means and roles under one roof. Its name sounds technical; its everyday work is not. If you can ask with whom, by what means or as what, you are already close to the point.
After с meaning with, the companion takes the instrumental: с другом, with a friend; с сестрой, with a sister. This is distinct from с meaning from/off, which takes the genitive. The ending and context make the relationship clear.
Russian can place a tool directly in the instrumental without an extra word: писать ручкой, to write with a pen; ехать поездом, to travel by train. The case answers by means of what? and often replaces an English with or by.
After verbs such as стать, to become, professions and roles commonly take the instrumental: Она стала врачом, she became a doctor. This use feels abstract at first, so store it as a verb pattern rather than forcing it into the tool meaning.

С + instrumental means with: с сестрой, с кофе, с большим удовольствием. But с + genitive can mean from or off: с работы, со стола. Context and ending separate the relationships. For beginners, storing a few complete contrasts is safer than assuming every с leads to the same case.
Писать ручкой means write with a pen, while ехать поездом means travel by train. After стать, a role often appears in the instrumental: Она стала врачом. English uses different prepositions or none at all, so literal translation is a poor guide. Attach the case to the Russian verb or meaning that summons it.
Close the English column and say кофе с молоком from memory. Change one useful detail, then answer yourself with ехать поездом. Record the two lines once at normal speed. Listen for the stressed syllable and for any ending you swallowed. Now wait five minutes and rebuild both lines without reopening the page. This is a small test of Russian instrumental case, but it measures the useful thing: can you retrieve the language when the prompt is no longer sitting in front of you? Finish by writing one sentence that is true about your day. Tomorrow, begin with that personal sentence before looking at either model. Change the person, time or place and listen for the point where the Russian form must also change. Keep the correction beside the sentence, not in a separate rule list. After three days, use the strongest version as the opening line of a voice note. If it arrives without rehearsal, the pattern is starting to belong to you.
